Submit a CMI Account Creation Form
- For instrument training and access, we'll follow up with instructions for getting setup in our PPMS booking system
- To request Data Collection Services or Nanobody Services, submit a preliminary CMI Sample Form

EXTERNAL USERS will also need:
- a signed CMI User Agreement
- a signed Harvard Risk and Release Form, if you will need a sponsored HUID for instrument access

a Purchase Order to cover expected CMI charges (a standing PO is recommended)
- External Users requesting Nanobody selection services (academic users only), require a special Nanobody Services Agreement that must be signed by the User, their PI, and by an Institutional Signing Official.
