CMI Account Creation

To request an account at the CMI, interested users must:

  1. Read and agree to our CMI User Policies and understand and agree to the access fees.
  2. Fill out the Contact Form below and "Request an Account at the CMI."
    • All External Users are required to also submit the appropriate CMI User Agreement, which you may upload in your account request form or send by email. External users who are not from an HMS-affiliated institution, will also need to submit a Harvard Risk and Release form.
  3. You will be notified with further instructions for setting up an account in the CMI PPMS booking system.
    • Requests from Internal (HMS) users are usually processed within a week.
    • It can take longer to create required sponsored IDs for External Users.
