User Agreements

  • All Users from External Labs (academic and commercial), who are requesting training and instrument access or services, are required to submit a signed CMI User Agreement.  
  • Users unaffiliated with Harvard Medical School, must also submit a signed Harvard Risk and Release Form for building access.  

Please Note:

  • We DO NOT accept any modifications to our standard user agreements.  

     

External Academic Users

  • Users from academic institutions affiliated with Harvard Medical School (BCH, BIDMC, BWH, DFCI, MGH, Ragon) need to submit a CMI User Agreement.
  • Users from unaffiliated academic institutions (BU, Tufts, UMass, Brandeis, etc) need to submit both a CMI User agreement and a Harvard Risk and Release form.
  • The "Institution" in some user agreements is the Company and should be signed by an institutional signing official (consult with your PI or your legal department, if unsure).

Academic User Agreement for Nanobody Services 

Required user agreement for for External Academic Labs requesting Yeast Surface Display Nanobody Selections. A new agreement is required for each selection project (but a project may include more than one antigen).

Commercial Users

  • The "Client" in the commercial user agreements is the Company and should be signed by an institutional signing official (consult your director or your legal department, if unsure who has this authority).
